COUNTY NEWS RELEASE

  • Public Health Service, PHS Form 1867.

To demonstrate proof of residency, veterans should provide one of the following:

  • A valid New Mexico driver’s license.
  • Voter registration card.
  • Most recent state income tax return.

If filing for a disabled veteran tax waiver, a Veterans Affairs award letter declaring 100% permanent and total service-connected disability is required.
