County: Pfizer Vaccine Clinic For Ages 5-11 Set For Monday, Nov. 22

COUNTY NEWS RELEASE

The New Mexico Department of Health (NMDOH) is administering the Pfizer vaccine for children 5-11 years old this Monday, Nov. 22 at the First Baptist Church, 2200 Diamond Dr., from 1 p.m. to 4 p.m. Appointments required.  

New Mexico Department of Health (NMDOH) has set up instructions at https://vaccinenm.org/kids for parents to register dependent children for the vaccine. 

Parents/Guardians should schedule appointments and download consent forms. Use the event code ESVLGNF when registering. 

Appointment holders for the clinic are reminded to: 

  1. Complete the online NMDOH medical questionnaire the morning of the clinic 
  2. Complete and bring the Parent/Guardian Consent Form  
  3. Arrive within 10 to 15 minutes of the appointment time 
  4. Wear a mask
